ないことには attaches to the negative plain form of a verb and means that without that action happening, something else cannot happen or will not go well. The main clause is typically a negative expression such as できない, 始まらない, or うまくいかない, so the pattern literally sets up 'if not A, then not B'. It carries a conditional nuance of 'unless'.
